There are times when Home Health Care is prescribed to alleviate the need for hospital admissions. Our services are dedicated to meet the needs of our patients and their families. Because home care and home health services under the Medicare Program are different, our Registered Nurses will be able to determine what services you need and qualify for under the many programs available. Our Registered Nurses and Home Health Care Coordinators work with case managers from your local Area Agency on Aging and discharge planners to help you receive the best service available that fits your individual needs.
Home Health Care is skilled nursing & health care that an individual can receive in their home for the treatment of an illness or injury. Our services can provide support from the following healthcare professionals:
Registered Nurse- (RN) Case Manager for the client in their home. The Registered Nurse will act as the liaison between the home care agency, case managers from the area agency on aging , hospital and faculty, discharge planners, the physician, and the caregiver. The RN will establish a care plan according to the Physician's orders. The RN will also provide specific skilled care treatments; educate clients' and their families, while helping to administer medications, and communicate/supervise the nursing assistant.
Licensed Practical Nurse- (LPN) Helps report change in clients status, works under the supervision of a nurse.
Home Health Aides- (HHA's) The Home Health Aide will help with activities of daily living which include: bathing, dressing, ambulation, simple physical exercises.
Homemakers/Companions- Will provide non-skilled services to clients in need of light housekeeping, laundry, errand running, shopping, meal preparation.
Physical, Occupational, Language Therapists- Will provide specific skilled care treatment for an array of diseases & disabilities. To help promote functionality.
Social Worker- Helps refer clients to community services and is an additional geriatric resource.
